I can also adapt to any environmental changes. Per my observation, I think the second image is doing the right thing.this is because when working on humans you need to make a thorough examination before diagnosing.so the nurse going through the card before looking for the best option is doing a great job. Also we need not to be in a rush when discharging our duties. We need to look for the best alternative. But in the first picture, she is acting without any consult and I think there might be a mistake in her duties which will not bring progress

I have been working as a care worker in Eleanor Nursing and Social Care Limited in Bexley Branch since 2023 A.D. In my opinion, the second nurse is doing task correctly because we should read and check the client's past history before we start the work. He seems that he is paying more attention to study and review the client's medical history that has mentioned in the record book. I think , he will start his care based on the previous history which was provided by the health care staff, that might be medication and any kinds of caring.

In my view I believe the second picture best depicts a professional worker and this is why; In my view, as a professional nurse one needs to to be well equipped and prepared as to how, when and what procedures needs to be taken when administering health care to patients and the second picture tells us that, the nurse in picture is carefully reading the medical history of the patient before he can proceed to executing of his duties.

My name is Sujan Karki. In my opinion both the nurses provided in image above are performing important nursing tasks correctly. The first nurse is carefully handling an IV bag, which is a critical aspect of patient care. The second nurse is focused on reviewing or charting patient records, which is equally essential in nursing practice. Both tasks are vital in nursing, but if I had to choose one as more central to patient care, I would select the first nurse handling the IV, as this directly impacts the patient’s immediate treatment and safety.

My name is Barsha Shrestha, and I am currently working at Bijayapur Hospital dharan in General ward department as a registered nurse from the past two and half years of experience in nursing, providing patient care and ensuring the proper administration of medical procedures. In my view, I think the nurse in the first image appears to be performing their task correctly. She is handling an IV drip with focus and precision, ensuring proper administration of medication or fluids. Proper IV management is crucial in preventing infections, air embolisms, or incorrect dosing. Therefore, the first nurse demonstrates better dedication to hands-on patient care responsibilities.

With regards to the above pictures, I would like to choose option B, the male nurse, who is conducting his task correctly. He seems as he is cross-checking the medication before administration. This is a critical step in ensuring patient safety, as it helps prevent medication errors. Nurse must check medicine doze, right patient, right time, right medication. In regards of option A, the female nurse is not doing her task correctly because she is trying to give fluid NS 100ml to the patient without wearing gloves which is not safe practice for both. At the time of medication or any procedure we have to use proper PPE such as mask, Gloves, Gown, face shield etc. as per situations to prevent cross infection.

My name is Zarak Isaac from Pakistan, I'm working for Medicare cardiac general hospital as Registered Nurse (Team leader) for 2 years in my opinion the female staff is performing the right task as she is giving fluid to the patient so the patient would get the hydration properly on time, or it would be any antibiotic dissolved in 100ml NS.

My name is Alberta Ohemeng, I am from Ghana. I am a practising midwife at Twifo Praso polyclinic, and I have been practising for almost 4 years. In my practice, I attend to pregnant women during their antenatal period. I also assist them in the delivering of their babies and help newborn babies who cannot breath at birth breath within the shortest possible time. I also monitor patients' vital signs and administer medications to patient. Outside my health care duties I enjoy cooking and sewing. With regards to the above pictures I will choose and talk about the first one that is the female nurse. I believe the female nurse is doing her task correctly. She is setting up IV fluid which is very important in the health care settings. Hydration is part of treatment to eliminate dehydration. The nurse is not just setting up IV fluid but she is also at the client's bedside monitoring the flow of the fluid .

My name is Naomi Akuliga, I am from Ghana and I am a nurse at the Seventh-day Adventist Hospital, where I have been working for over four years. In my role, I am responsible for providing comprehensive care to patients, including administering medications, monitoring vital signs, assisting with daily activities, and coordinating with other healthcare professionals to ensure the best outcomes for our patients. Outside of work, I enjoy reading, spending time with my family, and volunteering in community health outreach programs. I am passionate about continuous learning and am currently pursuing further studies to enhance my skills and knowledge in the nursing field. Also, with rewards to the two pictures shown above,I have chosen image 2 that is the male nurse,as the nurse who is conducting his task correctly. In Image 2, which happens to be the male nurse ,he is seen cross-checking the medication before administration. This is a critical step in ensuring patient safety, as it helps prevent medication errors. Cross-checking involves verifying the patient’s name, the medication, the dosage, the time, and the route of administration, which are essential components of the “five rights” of medication administration. By taking this precaution, the nurse reduces the risk of adverse drug events, which can have serious consequences for the patient. On the other hand, in Image 1, which is the female nurse , she is seen administering medication without wearing gloves and without performing proper checks on the medication. This practice is concerning for several reasons: 1. Infection Control: Not wearing gloves while administering medication can increase the risk of transmitting infections. Gloves are a fundamental part of personal protective equipment (PPE) and help protect both the patient and the nurse from potential contamination.2. Medication Errors: Failing to perform proper checks before administering medication can lead to serious errors. Without verifying the correct medication, dose, and patient, the nurse risks giving the wrong medication, which could cause harm or even be life-threatening. Colleague Comments Analysis. Comment 1: Comment: “I think we should always double-check medication dosages before administration to avoid errors.” Analysis: This comment underscores the importance of medication safety in nursing practice. Double-checking dosages helps prevent medication errors, which are a significant cause of patient harm. This practice aligns with the principles of safe medication administration and should be encouraged as it ensures the accuracy and safety of patient care. Comment 2: Comment: “Sometimes it’s okay to skip hand hygiene if you’re in a hurry and need to attend to a patient quickly.” Analysis: This comment reflects a dangerous misconception. Hand hygiene is a critical component of infection control and should never be skipped, regardless of time constraints. Proper hand hygiene protects both patients and healthcare workers from the spread of infectious diseases. It is essential to always follow hand hygiene protocols to maintain a safe healthcare environment. Comment 3: Comment: “We should involve patients more in their care plans to improve their engagement and outcomes.” Analysis: This comment highlights the value of patient-centered care. Involving patients in their care plans can lead to better adherence to treatments, improved satisfaction, and overall better health outcomes. It empowers patients, making them active participants in their own care, which is crucial for achieving positive health outcomes.

I'm Joseph Abuuro, a registered community health nurse from Ghana with 3 years of experience in providing health services in the community. As a dedicated and results driven community health nurse, I seek a challenging role in health care where I can utilise my skills and experience to drive good health and growth. I conduct home visits and health assessments for patients with chronic illness, I conduct growth monitoring of babies 0-5 years which includes vaccination, weighing and counselling, I facilitate community outreach programmes and health promotion activities. I choose the female in the sense that she seems alert focus even though I do not know if she attending to a patient or not. At least her eyes are open to anything happening at the ward. The man to me eyes seems to be closed and cannot see any happenings on the ward. As a nurse, you need to always be ready to help your patients using all your senses and not to have eyes closed during working periods.

Currently serving as Nursing Officer, I have gained valuable expertise in delivering exceptional patient care, managing complex medical cases and collaborating with members of the multidisciplinary team. My experience encompasses a wide range of healthcare settings including hospitals, clinics and community outreach programmes. Throughout my career, I have demonstrated a strong commitment to patient advocacy and safety. I have a proven track record of effectively managing patient data, maintaining accurate documentation and adhering to strict confidentiality guidelines. I also keep myself up-to-date with current trends in nursing practice by attending professional workshops and conferences. With respect to the above images, the female nurse seems to be focused and concentrated on the task which I agree with my colleagues who share that opinion but administering IV infusion without wearing gloves is unprofessional as the risk of contamination and infection is high. The male nurse on the other hand looks sleepy and out of focus which is not the right thing to do as a nurse while performing any given task. According to Agnes Asante, she assumes the female nurse might just be checking the IV fluid and not necessarily administering but to me the position of her fingers at the point of insertion can introduce infections if its not washed properly. In my opinion both of them seem not to be doing the right thing as professional nurses.

My name is Roger and I am a nurse working in Durban. I have worked there for 16 years and enjoy the interactions with patients and seeing them return home with instructions that will ensure they recover well and live a long and happy life. I think my choice is correct as he is steadying his hand as he reads the patient data, he doesn't want to get confused or mess up a patient's medications. Better to be safe and thorough rather than guess and fudge up the recovery. Reading is a form of communication and it is necessary to be correct as a lack of communication skills can be the difference between life and death.

I am in the person of Agnes Yeboah Asante, a registered general nurse in Ghana with years of experience in various departments which includes Medical, Surgical and Psychiatric unit. Currently, I have been assigned to the Emergency Room/Unit (ER) where attend to clients at the acute phase of their illness. Overall, my primary responsibility at the ER is prompt identification of medical concern, identify its severity, making the right decisions and intervening safely. This is done by triaging clients using the color code system to know who attend to first, start intravenous lines (IVs), administer medication per physician orders, give stitches as well as setting of broken bones. Personally, I have an extrovert personality which helps me work well with others and navigate social spaces with ease. Nursing has been a rewarding and fulfilling career and hence I always look forward to career advancement and growth. With regards to the two nurses I choose the female nurse, alert and present whereas the male nurse has his eyes closed which is suggestive of someone sleeping. Being sleepy or drowsy at work is a recipe for disaster, it puts the client in harms way. I do not share the same perception with some of my colleagues who believe the female nurse needs to be gloved, she is not touching the spike neither is she touching or inserting a canula. It might be that she is merely handling the IV bottle or cross checking with the chart. Secondly, some insist the male nurse is not sleeping, yet his head is extensively flexed away from the chart he is supposed to be reading with his eyes evidently closed as compared to the female who equally has her head bowed. However, I do agree that its imperative for nurses to check the patient's chart and plan of care before administering a medication/

For I think the male nurse is donig the right thing because before any procedurd of drug administration you have to check the file first for the six rights, therefore the file is the first thing to check for the patient's name, the time of drug administration, the right drug before going to the patient bedside and confirm your information.

With my 4 years of experience in nursing, I've come to believe that patient-centered care is the most effective.I've worked in various settings, like the Theater room and the neonatal unit. I've always ensured that I'm intimately aware of a patient's situation and their doctors' approach, so that I can help explain it if there's any confusion. Most recently, I've had experienced quite a number of difficult cases to diagnose. My diligent work, both inside and outside the lab, helped doctors make diagnoses quicker, ensuring that timely interventions were possible. With respect to images, I can observe that the lady is working efficiently as she is more confident and upstanding while checking the right IV medication to be administered. On the other hand, the man looks exhausted and is sitting on the bed's side rail which is unacceptable in our nursing profession.

I work in collaboration with the multidisciplinary terms and l am very observant. A nurse must be observant and alert. The female nurse is checking medication on patient treatment sheet before administration as l saw that she is beside patient bed while the male nurse eyes were closed even though he is having sheet in front of him. It seems he is not doing anything which is not right in nursing profession.

My name is Daniel Osei Sekyere. I completed my 3years Diploma in Nursing Course at S.D.A. Nursing and Midwifery Training College in 2016 and have 4+ years of experience successfully providing person to person patient centered care. I have worked for 2 consecutive years in a 10-bedded accident and emergency unit, a year in a general adult male ward and presently the interim charge nurse in the pediatric unit of the same secondary facility ( Father Thomas Rooney Catholic Hospital ) in Ghana. I am a respectful, compassionate and highly motivated person with excellent communication skills which includes verbal and non verbal, writing and listening skills. I am able to work effectively and productively in the multidisciplinary team and also able to organize and prioritize my own workload without supervision. I also have immense competence in practical aspects of venipuncture and well vexed in cannula placement inside veins to provide venous access. I possess basic I.T skills with Proficiency in Microsoft Office, Word and Power Point and able to type not less than 70 words per minute daily at the work place. Taking patient and family history is a daily activity as well as the administration and managing patient medication at the work place. As an interim charge nurse, it is my duty to critically assess every patient that frequents my unit, plan for his/her care including their safety. I am almost done with my UK NMC registration process with a PRN of 1022949480 pending OSCE. I have sat for IELTS with an overall band score of 7.5 and also have a pass in CBT-Adult nursing. Pertaining to the images above, i believe the female nurse is doing her task correctly. One quality of a good nurse is been observant. She is alert and seems to be reading from either a medication chart or patient notes in order to be sure about the drug in hand. The male nurse on the other hand looks tired and exhausted. He has his eyes closed even though there is a chart in front of him which to me is of no use at that particular time

My name is Abigail Abrafi Poku,a Senior Staff Midwife from Ghana.I had my training at the Nursing and Midwifery Training College,Sekondi in the Western Region of Ghana.I have worked for 5 years where I've gained clinical experience and skills.I am dedicated, committed, goal oriented and team player.Pregnancy is not a disease and as such no woman should die from pregnancy and labour complications that is why delivering high standard quality care to my clients is my number one goal.As a senior Staff Midwife, some of my duties are ;assessing, planning and implementing care(from pre-conception to conception), identifying high risk pregnancies and making prompt referrals, conducting skillful deliveries, identifying and managing obstetric emergencies such as PPH,Eclampsia,Placenta abruptio,APH and so on,educating client on breastfeeding, diet, nutrition and rest, foetal monitoring and so on.I help my clients make an informed decision about their care.I am competent in IV cannulation,Perineal Suturing,Blood transfusion,IV medication administration and venepuncture. From my observations, the male nurse is carrying out his task correctly. He is reading patient's chart so that he will be able to assess and plan appropriate care for his patient.I do not agree with my colleagues that are saying the male nurse is sleeping whiles he appears to be reading a chart.First of all,if one bends the head and lower the eyes to look down, it'll appear as though the eyes are closed especially when they're not blinking.Secondly,it is unprofessional to sleep whiles on duty and doing so can put one in great trouble which I'm sure the male nurse is aware of. I again agree with my colleagues that, think the female nurse doing the task wrongly is administering an IV medication without having a glove on.I second them because the risk of contamination is great especially since we can't also tell if she washed her hands.

I am Emmanuella Gyamera Appiah from Ghana. I have completed a Diploma in Nursing (3 years) from the Nursing and Midwifery Training College, Kumasi in Ghana. I am a registered nurse with over two years of strong clinical professional experience in delivering quality care to patients with profound knowledge of administration and assessment In every nursing role I have held, I have demonstrated a high degree of professionalism, a strong work ethic, and a commitment to maintaining the highest standards of clinical quality, patient care, and regulatory compliance. My responsibilities as a staff nurse includes, assessing the needs of the patients on the ward and making nursing care plans for all patients, giving direct patient care such as bed making, changing of bed sheets, mouth care, back care, bed bathing, hair washing, changing of position, fulfilling all basic needs like hygienic and nutritional needs of patients, establishing and maintaining long-term quality relatiohship with patients and their relatives through effective communication, working collaboratively with dieticians, and other healthcare teams, educating patients on their disease, treatment, and care plan to enhance their quality of life and promote a positive attitude. From my observation I would say that the male is doing his task correctly because he’s taking a critical look at the patient’s chart in order to avoid mistakes and work diligently.Comfort Mantebea made mention the female nurse is doing a drug check but from my observation I can’t see her with any chart to compare with the drug administration. Also I second to Rosemary Jackline’s comment because the female nurse is performing a procedure without wearing gloves. Again, from Gloria Ohene Darko’s comments I don’t think it looks realistic some people can be reading and you may think they are asleep.

My name is Comfort Mantebea and I’m 38 years old. I am a Ghanaian and currently living in Ghana. I am a nurse by profession and holds Degree in Critical Care Nursing which I obtained in the year 2020 at University of Cape coast located in the central region of Ghana . I have 14 years experience as a nurse . I speak english fluently and Twi which is a native language. My key responsibilities are as follows: Responsible for rendering quality health care, triaging, preventing illnesses, restoring and promoting health. Again, I am responsible for providing health education to people in the communities and ensuring that health policies are implemented as directed to promote the wellbeing of the ordinary Ghanaian. Assessing, developing, monitoring and implementing complete care plan through interdisciplinary team process for all admissions/transfers of patients in both medical and surgical fields. As a member of a team of nurses that worked overtime to accommodate the work overload for the hospital to achieve the best. I also manages the emergency department as the nurse manager to receive emergencies, triage emergencies, and stabilize for further management. My short term goal is to is to work in a more challenging environment to transform energy and attitude and also add efficiency in providing quality health services to my clients. My long term goal is to reach the topmost ladder in my career. I like listening to music, reading novels and watching television. From my observation , I deduce that the female nurse is administering medication by observing the rights. At least I can see her cross checking the medication with a chart to be sure unlike the male nurse who looks as if he is sleeping or only checking on the chart without the medication being present with him.

I’m Rosemary Jacqueline Mawulolo Dzandu by name , a nurse in Ghana.I have Bsc in Public health nursing which I acquired at the University of health and allied Sciences , Ho in Ghana.I have three years of experience in Public Health Nursing with a year’s of experience in the general or medical setting.I am a compassionate nurse who prioritizes patients thus regarding them with respect and dignity, I also communicate effectively with patients and other members of the health care team.I work effectively and provide safety in discharging my duties as well. I am highly organized with good time management skills, a keen and accurate observer , I possess good communication skills, both spoken and written.I also have good organizational and social skills.Furthermore , I have excellent team working skills, leadership skills relating to coordination and mobilization - I am thus the In-charge of the Reproductive and Child Welfare Clinic in my current hospital.I possess basic I.T skills as well. A brief description of what I generally do at my workplace are ; • Checking of vital signs • Catheterization • IV cannulation • Administration of medications (oral, intravenous, suppository, intramuscular, subcutaneous, intradermal) • Bed making • Peri- Operative and post-operative care • Conducting patient assessments and triaging based on the urgency of their condition. •Administering medications and treatments as prescribed by doctors. •Assisting with minor procedures and wound care. •Monitoring vital signs and observing patients for any changes in their condition. •Collaborating with other healthcare professionals to develop and implement patient care plans. •Educating patients and their families on managing their health conditions and medications. •Providing emotional support and reassurance to patients during their visit. • Documenting patient information accurately and maintaining medical records. •Assisting in emergency situations and providing immediate care as needed. •Participating in ongoing training and professional development to stay updated on best practices. • Family planning service, general health education, child welfare clinics, conducting home visits, TB management, child immunization activities (EPI) and report wiring and documentation. • Provided patients with post-discharge follow-up and recommended available counseling programs as needed. • Performed assessments for each individual case to determine which materials and services were the best fit. • Attended functions within the community and spoke with members of the public and local officials to promote methods that reduce illness, food contamination and obesity. • Oversaw data queries and made sure that they were rectified in the most efficient manner. • Tasked with identifying unusual issues, cluster infections and known pathogens on patient records, ensuring outbreaks were remedied in a timely fashion. • Performed outreach work with local homeless population to treat conditions and improve health outcomes. • Instructed patients and family members on daily tasks and care needs to improve continuity of care. From my observation, I see the female nurse in the first image is performing a procedure without donning gloves , the second nurse who is wearing two wrist watches seems as though he is sleeping on duty . Sleeping on duty is also not right .Looking at the risks involved in both cases and the ambiguity as to whether indeed the male nurse is sleeping or not , I would say he is doing the correct thing. Analyzing the comments made by my colleague Doreen Biitir , about the nurse not paying attention to what she is doing and looking elsewhere is not really obvious and about Evans Akwaboah stating the nurse is just examining the drugs and should not wear gloves is not the best.According to Gloria Ohene Darko, the male nurse is doing the right thing because “it looks like he is sleeping” but what if he is actually sleeping on duty ?.

My name is Doreen Damjat Biitir. I am a registered general nurse with 3 years direct patient care experience in a tertiary healthcare environment. I’m knowledgeable in handling both acute and chronic medical/surgical conditions and emergencies. I provide person centered care with patient safeguarding being my outmost priority. I have the ability to work under pressure whiles maintaining patient’s safety, I am proactive and adapt quickly to change and always eager to learn and develop new skills and knowledge in my line of work. My answer is none of them is performing their task correctly because lady is not paying attention to what she doing at patient’s bedside and looking else where and also not in glove and the man sleeping whiles on duty is not allowed on duty. Evans Akwaboah made a comment on negligence from the man’s picture which is a great point because as a nurse you have a duty of care and that is to adhere to standard precautions to avoid carelessness which can cause which sleeping on duty can ‘cause negligence because you can’t monitor the ward and be sleeping. Also Gifty Frimpomaa raised the issue of right on drug administration. Yes before serving any medication you need to check for the right dose, right time, right route, expiration which you cannot do so by using just the patient but also have your medication at hand so you compare the route, dose and expiration to the manufacturer’s instructions. Lastly Winifred Sarfo made a comment on Documentation which is very important in our field. Documentation helps in continuity of care and also serves an evidence to your claims to defend yourself in events of legal issues.

I currently work in a medical and surgical ward as well as a hemodialysis unit where I assess, plan, implement and evaluate patient care from admission to discharge, prioritizing patient safety and dignity and upholding the ethics of the Nursing and Midwifery Council. I think the gentleman is not doing the right thing as examining patient chart cannot be done with eyes closed because it is negligence and unethical to sleep on duty. On the other hand, the lady is connecting a giving set to an IV Drug/ Fluid but she is also not in gloves which exposes her hands for possible contamination of the drug, which is not right. Unless that is not the activity she is doing but just examining the drug.

I am responsible for my own time management and that of my colleagues. I ensure the smooth running of the ward by prioritizing, delegating and supervising activities based on level of competencies. I also organize my own workload and that of others. I work well under pressure and take responsibility for a caseload of patients as required. I am accountable for assessing, planning, implementing and evaluating high quality evidence-based care and maintain accurate and complete documentation. I genuinely care about the well-being of my patients and I am dedicated to making a positive impact in their lives. The second image shows a male nurse with a document that appears to be a patient’s chart. I believe this action is correct because as a nurse, documentation happens to be very vital in our line of work and it is very important to cross-check every document prior to or post every action taken. Documentation ensures safety, continuity of care, and can also save the nurse from any medico-legal issues. Additionally, rights of drug administration, patients comfort, and patients education were a few other comments made by my colleagues which I believe are also very relevant in promoting high standards of care and would be necessary for critical analysis.

My name is Frank Gyamfi from Ghana. 31 years of age and single. I qualified as a nurse in 2020 with a degree in general nursing in 2019 from University of Health and Allied sciences in Ghana. My previous studies and experience in nursing (4years) has enlightened me to nursing as a holistic and a continuous process comprising of the social, emotional, physical and psychological needs of patient. I have assumed multifaceted responsibilities that equipped me with a comprehensive skill set. My primary duties include, overseeing patient care plans, conducting thorough assessments, and coordinating with interdisciplinary teams to ensure seamless healthcare delivery. I have been actively involved in implementing evidence-based nursing interventions, emphasizing a patient-centric approach. One of my key responsibilities was to supervise and mentor junior nursing staffs and students, fostering a collaborative and supportive work environment, coping with the emotional demands of working with patients and their families in distressing situations such as unexpected illness, injury, challenging behavior, life-limiting conditions, and critical care. I have chosen the nurse on my right hand (second picture), because he is doing the right thing, that is, doing the checks and observing the rights of medication administration. Isaac Kumah Tandoh said “I have paid my dues in serving the community through various outreaches and voluntary works done over time”. I agree with him to always give back to society. Benedicta Opoku commented “In the field of work my goal is to emphasize more on empathy that sympathy”. Truly, in health care environment, providing empathy to our dear patients is therapeutic. Obed Asante wrote “Confirming patient data before undertaking any task is crucial in nursing practices and vital for patient safety”. Ensuring patient safety is paramount in health care delivery and one way to do that is identifying your patient and so I agree with him.

These are some comments made by my colleagues which I always think about and put it into practice, such as made patient comfortable in bed before checking vital signs, observe all the right in drug’s administration and check to see if Nasogastric tube is in the right place before feeding a helpless patient.

I possess good communication skills, the ability to work as part of a team and who is willing to undertake additional training in order to give off my very very best in providing client centered care while upholding professional standards with compassion and respect. As a result oriented, client centered, individualized care nurse who upholds professional standards while promoting teamwork amongst colleagues in a multidisciplinary approach of rendering care, I have paid my dues in serving the community through various outreaches and voluntary works done over time. Over the years of working, I have gained various skill set which makes me unique and makes me stand out amongst my peers. That includes but not limited; effective and efficient use of resources, interpersonal relationships amongst colleagues, cannulation, Basic Life Support algorithm, effective communication, proper documentation and reporting, the use of various hospital administration software, etc. just to mention but a few. From the two images given above, I am inclined to the second image as proper checks are in order prior to administration of medications in order to prevent medication related errors which may be dangerous to the patient. There is no afore knowledge or evidence of the first nurse as to whether these checks were done prior to the administration. Again, 3 comments made by colleagues that needs my analyses include: 1. Patient lodged no complaint: I believe that if there was nothing wrong the patient, he/she won't still be on admission. Again the fact that there no complaints does not mean that the old problem has been solved. This can only be so if an evaluation of a patient centered plan is done. 2. vital signs checked and recorded as per chat: what were the values? was there any deviation from the norm| if so what was done about it? 3. Patient was made comfortable in bed: comfort is relative: where comfortable mean? is the position being called comfortable for the patient detrimental to his health?

I think the second image is doing the right thing because I observed the ward is empty but he is reading through notes to equipp himself or probably cross checking to see if everything was done right and what is left undone Pertaining to the comment by colleague looking through patient records is very crucial in other to know their condition and want needs to be done for them Also, cross checking the what needs to be done and doing same after completing it saves you from a lot of trouble and helps you avoid mistakes and erros. In addition, documentation is essential in every activity you do for the patient. It important you always document each procedure and findings.

As a registered midwife my duties are to assess pregnant and pueperial mothers , to detect and manage potential risk at the prenatal and postpartum stages in both the mother and the baby. Also I Offer emotional support and counseling to expectant mothers and their families addressing any concerns or questions regarding the pregnancy. In the field of work my goal is to emphasize more on empathy that sympathy. With respect the images I choose the second person to be doi With respect to the images I see the second person to be doing the right thing because he is glancing through the patient's bedside to get insight on the patient's condition as well as the medications he or she is been managed with. Furthermore, he is cross checking to ensure that the right medication will be administered at the right time using the right route and the right dosage. However, image one wasn't selected because the nurse was administering the medication without putting on gloves Regarding the comments by my colleagues which needs to be analyzed; 1)" Do not persuade a client to accept" treatment. In as much as a client is not to be persuaded, you have to let the client know the risk involved if she rejects the treatment she's been given. 2) "Medication will not be given as it is pregnancy related". Some pregnant women report with genuine complaints that if only thoroughly investigated will come to light that it is detrimental to their health. 3)"This needs not to be documented" As a health worker we are very aware of the significance of documenting every detail and procedures done on patients purposely for future reference and continuity of care.

Some of responsibilities as a midwife in the unit includes admission and transfer of clients, checking and recording of vital signs, auscultation for fetal heart rate, receiving babies from the theater, administration of medications (oral, intravenous, intramuscular and subcutaneous), assisting mothers in breastfeeding, assessing the newborn and providing essential care to newborns among other duties. I have 3 years experience in providing prenatal, antenatal and postnatal care to diverse populations. I am knowledgeable about the neonatal intensive care unit and emergency procedures for newborns including resuscitation. After assessing the two pictures, I believe the nurse(male) in the second picture is the one doing the task correctly. This is because, he is observing the rights of drug administration. Moreover, the nurse(female) in the first picture is administering an intravenous medication without a gloved hands. Administration of I’ve medication is a sterile procedure and should therefore be done as such.

I go by the name Isaac Kumah Tandoh, an very enthusiastic registered nurse from Ghana with 5years worth of experience in beddsde nursing as well as 5yeras experience in Nursing tutorials accumulated a s part time nurse educator.

My name is John Honny, an internationally trained Registered Nurse with over 3 years working experience. I am self motivated individual who is committed to rendering holistic nursing care to diverse patient population and serving them with dignity. I possess the requisite skills and qualities that make me an ideal candidate for most nursing roles. Some of my responsibilities are; Triaging and stabilizing of incoming patients. Admission and discharge of patients. Monitoring and recording of patients vital signs. Caring for patient with care plan approach. Wound dressing. Management of hyperglycemia and hypoglycaemia. Setting up and securing IV lines. Administering oral, intravenous, intramuscular, subcutaneous and suppository medications. Establishing therapeutic relationship with patients. Performing minor procedures such splinting, suturing and caring for uncomplicated skin lacerations. Looking closely at both images, I think the nurse in the second image is doing the task correctly because he is observing some of the rights in drug administration (Comfirming the order, taking notice of what has been prescribed). On the other hand, I personally think the setting up an IV infusion is a procedure which requires decontamination of the hand and wearing of gloves.

I'm Blake Williams a registered nurse from Ghana who completed school of nursing and midwifery training college Korle-bu in the year 2019. I'm currently working at medical department of Ussher Hospital. I have 1 year and 10 months working experience. I'm a dedicated patient advocate, a good communicator, a good team player and an expert multitasker when providing care to patient. I'm also hardworking, enthusiastic and reliable with a reputation of building great rapport between medical providers and patients. Some of my responsibilities include Administering prescribed medications, ensuring correct and appropriate doses. Administration of oxygen therapy. Monitoring and recording patient vital signs and reporting any changes in patient condition. Education of patient about their medical condition, prevention and management. Collection of patient blood sample for various requested laboratory investigations. I selected the second image and the reason is because the nurse is accurately going through the patient's chart to know the right medication, right time and right patient to care for in order to prevent errors. Some comments my colleagues passed which i analysed critically were know your patient. Knowing your patient is establishing good communication with the patient. This helps the nurse to understand how the patient experiences the present situation. Another comment passed was always recheck the patient chart when serving medications. To prevent errors one must ensure to recheck patient chart to confirm the right drug, the right route and right patient before serving medications. The last comment was ensure patient is safe. Patient safety must be prioritised. Patient must be protected and free from harm.

I respond to the name Irene Maame Pinaman Nimoh, a registered General nurse with 3 years working experience in Ghana. I am so much dedicated to my profession, able to work with a team and capable of working under little or no supervision. Some of my roles on duty are handing over and taking up of a ward, monitoring patients' vital signs, administering prescribed medications through the right route, educating patient on condition and medications prior to discharge, just to mention a few. The second image was chosen because it could be clearly seen that the nurse is going through a chart to make sure that the right drug or procedure is served or performed to the right patient at the right time to prevent errors. Some comments passed on by colleagues at work are; do not wish to know who a patient is before treating him/her right. Treat everyone equally the right way, ensure that all procedures to be performed on patient are clearly explained and well understood by patient before proceeding, make sure to document everything you observe and do on a patient. Task that are not documented are equally not performed.

My name is Regina Senakey,a registered nurse with four years working experience.I describe myself as a calm ,compassionate and a knowledgeable nurse who looks out for the wellbeing of the people around me. Some of my duties and responsibilities as a nurse at my work place are ;Checking and recording of vital signs, administration of drug, giving counseling and health education to patients and relatives, wound dressing, assessment of patients.etc Among the two pictures I have chosen the male nurse to be delivering his task correctly. This is because he’s first crossing checking the notes to ensure that the right medication or procedure is given to the right patient at the right time in order to avoid error and this is a good nursing practice that one needs to uphold. Lastly, three comments made by colleagues were ;always do the right thing at right time, always ensure that the right medication is administered to the right patient at the right time and also I should always document every procedure or task given to a patient.

I am a good team player and have the zeal to work and learn from my othe colleagues. I have a lot of responsibilities such as doing patient assessment, checking vital signs, feeding off helpless patients, wound dressing, administration of medications, education of patient and many more. From the two images above, I have chosen the second one to be doing the right thing because he is doing cross checking to see that if it is the right patient and the right medication or procedure that is to be carried on patient at the right time. These are some comments made by my colleagues which I always think about and put it into practice, such as made patient comfortable in bed before checking vital signs, observe all the right in drug’s administration and check to see if Nasogastric tube is in the right place before feeding a helpless patient.
